This is an application for a license to operate paper gaming packets as defined by Chapter 132 of the Code of Public Laws for Allegany County, Maryland, pursuant to authority granted by Section 13-201 et sec of the Criminal Law Article of the Annotated Code of Maryland Annotated Code of Maryland.

Before this application will processed, the applicant (or other person where indicated) must:

  • Fill out the application completely. Any missing items will prevent the application from being processed.
  • Attach a notarized PAPER GAMING LICENSE APPLICATION PRIOR RECORD AFFIDAVIT for the applicant, if applicant is a sole proprietorship; for each partner, if the applicant is a partnership; or for each officer, if the applicant is a corporation, unincorporated association, or other legal entity.
  • Attach an enlarged copy of the driver’s license for each officer or representative associated with the organization.
  • If the applicant is subject to tax, attach copy of paid tax bill for the current fiscal year or Certification from the State, County, and municipality (as applicable) that all real and Personal property taxes (including current fiscal year) have been paid.
  • Attach a copy of the special class “C” liquor license obtained for the event
  • Attach an IRS exempt letter or evidence of non-profit status as stated on application
  • Attach the TEMPORARY OPERATOR’S PAPER GAMING LICENSE APPLICATION STATEMENT. Be sure it is completed and signed.

Please type or print (in ink) the application. Illegible application will not be accepted.

FALSE, OMITTED, OR MISLEADING INFORMATION PROVIDED IN THIS APPLICATION SHALL BE JUST CAUSE FOR DENYING A LICENSE OR VOIDING A ISSUED LICENSE.

CHECKLIST FOR TEMPORARY PAPER
GAMING LICENSE APPLICATION

  • Application forms completed.

  • Social Security number and daytime telephone number for each officer and/or representative. 

  • Completed and notarized PAPER GAMING LICENSE APPLICATION PRIOR RECORD AFFIDAVIT for each officer or representative or the organization.

  • Enlarged copy of driver’s license for each officer or representative.

  • A copy of the special class “C” liquor license obtained for the event.

  • IRS tax exemption determination letter, other evidence of non-profit status, or signed statement of purpose and date of existence of organization.

  • A TEMPORARY PAPER GAMING LICENSE APPLICATION STATEMENT – signed.

The completed application, with all required attachments should be submitted to:
