Operator's Paper Gaming License
Application Procedure

This is an application for a license to operate paper gaming packets as defined by Chapter 132 of the Code of Public Local Laws for Allegany County, Maryland, pursuant to authority granted by Section 13-201 et sec of the Criminal Law Article of the Annotated Code of Maryland Annotated Code of Maryland.

Before this application will be processed, the applicant (or other person where indicated) must:

  • Fill out the application completely. Any missing items will prevent the application from being processed.
  • Attach a notarized PAPER GAMING LICENSE APPLICATION PRIOR RECORD AFFIDAVIT for the applicant, if applicant is a sole proprietorship; for each partner, if the applicant is a partnership; or for each officer, if the applicant is a corporation, unincorporated association, or other legal entity.
  • Attach an enlarged copy of the driver's license for the applicant, if applicant is a sole proprietorship; for each partner, if the applicant is a partnership; or for each officer, if the applicant is a corporation, unincorporated association, or other legal entity.
  • If the applicant is subject to taxes, attach copy of paid tax bill for the current fiscal year or certification from the State, County, and municipality (as applicable) that all real and personal property taxes (including current fiscal year) have been paid.
  • For restaurants and taverns for which an alcoholic beverage license has been issued, and holders of Class A beer, wine, or liquor license.
    1. attach a copy of current business license of applicant; and
    2. attach a copy of current liquor license of applicant.
  • For a religious organization, fraternal, civic, veterans, and charitable organization, attach a copy of your IRS tax exemption determination letter.  If you are not a tax-exempt organization, then attach evidence of your status (such as a copy of your charter) or a letter signed by your corporate officers stating your charitable purpose and the date of your organization.  A sportsmen’s association must submit a copy of its IRS Section 501(c)(3) determination letter.
  • Attach a completed and signed OPERATOR'S PAPER GAMING LICENSE APPLICATION Statement.
  • Submit to fingerprinting, at County Commissioners’ discretion, as outlined in the Allegany County Rules and Regulations.  (All fingerprinting shall be at the discretion and under the supervision of the Gaming Office.)
  • Corporations, limited partnership, limited liability partnership, and limited liability Companies must be in “Good Standing” with the State Department of Assessments and Taxation.

Please type or print in ink the application. Illegible applications will not be accepted.

FALSE, OMITTED OR MISLEADING INFORMATION PROVIDED IN THIS APPLICATION WILL CONSTITUTE GROUNDS FOR DENYING A LICENSE OR VOIDING AN ISSUED LICENSE.

CHECKLIST FOR OPERATOR’S PAPER GAMING LICENSE APPLICIATION

  • Application form complete and accurate.

  • Social Security Number for each owner and/or corporate officer.

  • For restaurants and taverns, attach a copy of business license

  • Attach a copy of current liquor license.

  • Notarized PAPER GAMING LICENSE APPLICATION PRIOR RECORD AFFIDAVIT(S) for each owner and/or corporate officer attached.

  • Copy of driver's license for each owner and/or corporate officer attached.

  • Signed OPERATOR'S PAPER GAMING LICENSE APPLICATION STATEMENT attached.

  • Evidence that all taxes have been paid.

  • Copy of paid personal property tax bill.

  • Must be in “Good Standing" if a corporation.

  • IRS tax-exempt determination letters, other evidence of status or signed statement of   purpose and date of organization.

Special Note

Operator's Off-Premise Application
A subset of the Operator License.  An organization must have an operator's license to be eligible for an Off-Premises License
